FTC Disclosure and Disclaimer Form

Per the FTC Funeral Rule & Islamic Funeral Practices
Effective Date: 05/01/2025

1. General Price List (GPL)

Per the Federal Trade Commission’s Funeral Rule, you have the right to receive a General Price List (GPL) before discussing any funeral service arrangements.

• I acknowledge that I was provided a copy of the General Price List before any discussion of prices or services.

2. Casket and Outer Burial Container Price Lists

• I have been informed of the policy for the use of caskets and outer burial containers. If applicable, I have received the Casket Price List before viewing or selecting such merchandise.

• I acknowledge receipt or access to the Casket Price List.

• I acknowledge that the Outer Burial Container is not provided by Al Firdous Funeral Service.

• I understand that the law does not require you to purchase a Casket or Outer Burial Container.

3. Embalming Disclosure

Except in certain special cases, embalming is not legally required. Embalming is not practiced in traditional Islamic funeral rites unless legally mandated for transport or public health.

• I understand that embalming is not required by law and is not part of Islamic funeral services, unless legally necessary.

• I understand that embalming is not a service provided at Al Firdous Funeral Service.

• I do not authorize embalming under any circumstances.

4. Crematory Disclosure

• I understand that Cremation is not a service provided by Al Firdous Funeral Services.

5. Statement of Goods and Services Selected

You will receive a detailed Statement of Funeral Goods and Services Selected, showing the goods and services you have chosen along with the total cost. You are not required to purchase a package. You have the right to choose only the services and goods you desire.

• I understand that I will receive an itemized list of selected services and am not obligated to buy a package, or any services not required by law or religious observance.

6. Islamic Funeral Provisions

• I acknowledge that I have selected funeral services which implement the Washing, Shrouding, Janaza salat and Burial based on Sunni Islamic practices only.

• I acknowledge that I have selected funeral services consistent with Islamic practices guided by Sunni traditions, including:

  • Prompt burial, ideally within 24 hours
  • No embalming
  • Simple shrouding (kafan)
  • Use of a plain burial container or no casket, if permitted by local cemetery regulations
  • No public viewing, unless culturally requested
  • Ghusl (ritual washing) and Janazah prayer service

7. Disclaimer of Legal Requirement

Unless specifically stated, no service or merchandise described in our price lists or offered by our staff is required by law. Certain cemeteries may have their specific requirements.
